What is a Primary Care Network?
Primary Care Networks are where GP practices, communities, mental health, social health, pharmacy, hospital, and voluntary services in local areas covering around 30,000 to 50,000 patients work together. PCNs are intended to bridge a gap between primary and community services. Fleetwood PCN works together to provide coordinated care through integrated services to make sure that patients are the number one focus.
